We are recruiting a coordinating physician M/F to join an EHPAD located in Marly, in the Nord department, as part of a part-time CDI (0.4 FTE).
Description and duties
You will be working in a facility for the frail elderly, as part of a structured medical coordination program. As coordinating physician, your main duties will be to:
- Drawing up and updating individualized care plans
. - Coordination with in-house and external healthcare professionals
- Organization of the care pathway and admissions
>. - Coordination of the quality approach, risk management and care protocols
. - Coordination of medical care team
- Contribution to training and assessment of professional practices
